Acknowledgments

 This manual was first compiled by the CUDCP Board of Directors, and later revised by Sandra Harris in 1990.  Pat Wisocki provided helpful comments on prior drafts.  The section on legal issues is a revision from the 1990 manual for which Russ Newman provided important information.  Russ Newman’s talk on “Liability in Clinical Training” at the 1994 CUDCP meeting served as the basis for many of the revisions.  The appendix on clinical program handbook information was written by George Allen, Bev Thorn, and Sarah Bullard.  Cynthia Belar, Steve Dollinger, Bob Flynn, Cyndee Kalodner, Michael Perone, Mark Roberts, Mike Roberts, Rick Schulte, and Milt Strauss contributed forms and other materials that were used in this manual.  George Allen and Dave Hansen have been fellow presenters with me at Workshops for New DCTs for several years, and so provided many gems of wisdom that are included here.  Susan Zlotlow provided information about the confusing array of acronyms used at CUDCP meetings.  Many colleagues, including Steve Lisman, provided helpful ideas and encouragement along the way.  Amy Herschell, Michael Zvolensky, Karen Joseph, and Kevin Vowles, graduate students at West VirginiaUniversity, were very generous with administrative support in the construction of this manual.  I gratefully acknowledge Ann Davis, administrative secretary in the Department of Psychology at West Virginia University, and her many hours of careful typing and proofreading in helping to create the various recent versions of this manual.

 Cynthia Belar suggested the idea of a manual revision during her tenure as Chair of CUDCP, and encouraged me to take on this endeavor.  I thank her and my fellow CUDCP Board members for the opportunity, including our current chair, Bev Thorn.  My wife, Cheryl, and sons, Danny and Will, allowed me the time for this task; I acknowledge them with thanks and love. 

A draft version of this manual was distributed to all attendees at the 1997 CUDCP meeting; comments, sample materials, changes, and additions were invited.  A modified version was then produced in 1998,  and now again in the new millennium, 2000.  This 2000 version manual is the result of the contributions of many people over the last 10 years.  I hope that it is useful to my fellow DCTs and might in some small way contribute to helping faculty and students in the enterprise of Clinical Psychology training.
